If you were residing in the United States and your Private Information was potentially impacted by the Data Incident affecting Fujifilm, that Fujifilm discovered in approximately September 2025, then you may be a Settlement Class Member.
If you are a Class Member and wish to receive Settlement Benefits then you must submit a Claim Form by November 25, 2026.
All Class Members are eligible to enroll in Credit Monitoring and one of the two Cash Payment options. These benefits are described in more detail below.
Credit Monitoring. All Settlement Class Members will receive two (2) years of free Credit Monitoring Services from CyEx Identify Defense Complete. Enrollment instructions will be available after the Court grants final approval.
Out-of-Pocket Losses. Settlement Class Members may submit a Claim Form with reasonable documentation for losses related to the Data Incident for up to $2,500.00 per Settlement Class Member (subject to a pro rata adjustment based upon the total value of all Valid Claims). Eligible losses include those incurred on or after September 4, 2025, through the Claim Deadline and include (without limitation):
bank fees;
long distance phone charges, cell phone charges (only charged by the minute);
data charges (only if charged based on the amount of data used);
postage;
gasoline for local travel; and
fees for credit reports, credit monitoring, or other identity theft insurance products purchased.
You need to submit documented proof, such as (but are not limited to), telephone records, correspondence including emails, or receipts to show how much you spent or lost. You can also provide notes or papers you made yourself to explain or support other proof, but those notes or papers alone are not enough to make a valid claim. Your proof or notes should show that your expenses were because of the Data Incident.
You will not be reimbursed for expenses if you have been reimbursed for the same expenses by another source, including compensation provided in connection with the medical monitoring and identity theft protection product offered as part of the notification letter provided by the Defendant or otherwise.
Alternative Cash Payment. Instead of claiming for Out-of-Pocket Losses, Settlement Class Members may submit a Claim Form to receive an alternate cash payment in the amount of $50.00.

The deadline to submit a Claim Form online is November 25, 2026. If you are mailing your Claim Form, it must be mailed with a postmark date no later than November 25, 2026.
